For family doctors and interested parties: Calculate evolution of renal function yourself

Basis
The renal function calculator calculates the estimated creatinine clearance rate (CrCl) on the basis of the patient’s serum creatinine level and the patient’s age, height, weight and sex. The calculation is based on the Cockcroft formula (creatinine, age, sex), but also takes account of lean body mass (calculated from height, weight, sex). This gives a more accurate estimate, particularly for patients who are over or underweight.
The calculator also calculates the evolution of the renal function over time, and predicts the reaching of a CrCl of 15 ml/min or renal failure requiring replacement therapy (e.g. time of anticipated start of dialysis, CrCl 10 ml/min). The slope is represented graphically in different colours which reflect the anticipated period of time until renal failure (CrCl 10 ml/min): green > 3 years; yellow 1-3 years; red < 1 year.

Please note: The accuracy of the prognoses cannot be guaranteed, as the evolution of renal dysfunction varies individually depending on various factors. It should also be noted that meaningful values are given only if data for at least 3 different time points are entered and there is a clear trend for a deterioration in function.

Assessment and measures to be taken
- In the case of relevant impairment of renal function to an eCrCl < 60 ml/min it is recommended that additional investigations are carried out and/or that a nephrologist is consulted.
- In the case of rapidly deteriorating renal function with predicted renal failure in < 3 years, it is recommended that a nephrologist is consulted.
